PRIOR AUTHORIZATION CERTIFICATION EVALUATION REVIEW (PACER)

Reconsiderations The attending physician/dentist or the hospital may request reconsideration of the adverse determination of the ACRC regarding the need for admission, readmission, transfer, or continued stay. This reconsideration right applies regardless of the current hospitalization status of the beneficiary. Reconsiderations must be requested within three business days of the adverse determination. (Refer to the Directory Appendix for ACRC contact information.) If requested by the ACRC, the provider must provide written documentation. The provider is notified of the reconsideration decision within one business day of receipt of the request or the date of receipt of written documentation. If the initial adverse determination is overturned, the adverse determination is considered null and void. If the initial adverse determination is upheld or is modified in such a manner that some portion of the hospital care is not authorized, the hospital is liable for the cost of care provided from the date of the initial determination, unless this determination is overturned in the Medicaid appeals

If the ACRC does not authorize the admission or the continued stay for an admission and the beneficiary remains in the hospital for one or more days after Medicaid payment is not authorized, the hospital is at risk of Medicaid nonpayment for those days.     Rival countries have wrangled over territory in the South China Sea for centuries, but tension has steadily increased in recent years.

    China, Vietnam, the Philippines, Taiwan, Malaysia and Brunei all have competing claims.

    China has backed its expansive claims with island-building and naval patrols. The US says it does not take sides in territorial disputes, but has sent military ships and planes near disputed islands, calling them “freedom of navigation” operations to ensure access to key shipping and air routes.

    Both sides have accused each other of “militarising” the South China Sea.

    There are fears that the area is becoming a flashpoint, with potentially serious global consequences.

    What is the argument about?

    It is a dispute over territory and sovereignty over ocean areas, and the Paracels and the Spratlys – two island chains claimed in whole or in part by a number of countries.

    Alongside the fully fledged islands, there are dozens of rocky outcrops, atolls, sandbanks and reefs, such as the Scarborough Shoal.

    Why are they worth arguing over?

    Although largely uninhabited, the Paracels and the Spratlys may have reserves of natural resources around them. There has been little detailed exploration of the area, so estimates are largely extrapolated from the mineral wealth of neighbouring areas.

    The sea is also a major shipping route and home to fishing grounds that supply the livelihoods of people across the region.

    Who claims what?

    China claims by far the largest portion of territory – an area defined by the “nine-dash line” which stretches hundreds of miles south and east from its most southerly province of Hainan.

    Beijing says its right to the area goes back centuries to when the Paracel and Spratly island chains were regarded as integral parts of the Chinese nation, and in 1947 it issued a map detailing its claims. It showed the two island groups falling entirely within its territory. Those claims are mirrored by Taiwan.

    However, critics say China has not clarified its claims sufficiently – and that the nine-dash line that appears on Chinese maps encompassing almost the entirety of the South China Sea includes no coordinates.

    It is also not clear whether China claims only land territory within the nine-dash line, or all the territorial waters within the line as well.

    Vietnam hotly disputes China’s historical account, saying China had never claimed sovereignty over the islands before the 1940s. Vietnam says it has actively ruled over both the Paracels and the Spratlys since the 17th Century – and has the documents to prove it.

    The other major claimant in the area is the Philippines, which invokes its geographical proximity to the Spratly Islands as the main basis of its claim for part of the grouping.

    Both the Philippines and China lay claim to the Scarborough Shoal (known as Huangyan Island in China) – a little more than 100 miles (160km) from the Philippines and 500 miles from China.

    Malaysia and Brunei also lay claim to territory in the South China Sea that they say falls within their economic exclusion zones, as defined by UNCLOS – the United Nations Convention on the Law of the Sea.

    Brunei does not claim any of the disputed islands, but Malaysia claims a small number of islands in the Spratlys.

    Recent flashpoints

    The most serious trouble in recent decades has flared between Vietnam and China, and there have also been stand-offs between the Philippines and China. Some of the incidents include:

    • In 1974 the Chinese seized the Paracels from Vietnam, killing more than 70 Vietnamese troops.
    • In 1988 the two sides clashed in the Spratlys, with Vietnam again coming off worse, losing about 60 sailors.
    • In early 2012, China and the Philippines engaged in a lengthy maritime stand-off, accusing each other of intrusions in the Scarborough Shoal.
    • Unverified claims that the Chinese navy sabotaged two Vietnamese exploration operations in late 2012 led to large anti-China protests on Vietnam’s streets.
    • In January 2013, Manila said it was taking China to a UN tribunal under the auspices of the UN Convention on the Laws of the Sea, to challenge its claims.
    • In May 2014, the introduction by China of a drilling rig into waters near the Paracel Islands led to multiple collisions between Vietnamese and Chinese ships.

    Have they tried to reach a resolution?

    China prefers bilateral negotiations with the other parties. But many of its neighbours argue that China’s relative size and clout give it an unfair advantage.

    Some countries have argued that China should negotiate with Asean (the Association of South East Asian Nations), a 10-member regional grouping that consists of Thailand, Indonesia, Malaysia, the Philippines, Singapore, Brunei, Laos, Vietnam, Myanmar and Cambodia.

    However, China is opposed to this, while Asean is also divided over how to resolve the dispute.

    The Philippines has sought international arbitration instead. In 2013, it announced it would take China to an arbitration tribunal under the auspices of the UN Convention on the Laws of the Sea, to challenge its claims.

    In July 2016, the tribunal backed the Philippines’ case, saying China had violated the Philippines’ sovereign rights.

    China had boycotted the proceedings, and called the ruling “ill-founded”. It says it will not be bound by it.

    Establishing a New Claim

    If you become unemployed and have worked in Vermont anytime in the past 18 months, you may be eligible to receive unemployment insurance.

    Once you become totally or partially unemployed, the time to establish a new claim is during the first week you work less than 35 hours.

    An unemployment compensation claim becomes effective when filed, so if you choose to wait to file your claim, then you may not go back to the date you first became unemployed.

    How to File Unemployment Benefits

    When you apply, you will need to have the following information available:

    • Your Social Security Number or Alien Registration Number;
    • The business name, address and telephone number of each place you worked at during the past 18 months; and

Current unemployment claimants can view and print heir benefit history transcripts, view their claim status, manage the direct deposit of their benefit payment, reopen an existing account or file a weekly unemployment claim. New users can access the Maine Job Bank, mainecareercenter.com and file an initial unemployment claim. Users can obtain benefit history transcript, often needed when applying for services from other agencies, for example, LiHeap.

    Filing by Mail:

    To establish a new claim for unemployment (Initial claim) the quickest and most complete method is to file on the internet at www.file4ui.com. You will need to create your own web portal account and complete the initial claim process. The web portal provides a number of self-service options for you to manage your account.

    After you have established a new claim you must file weekly claims for unemployment for each week that you wish to file for benefits. You can file weekly claims on the internet through your web portal account. You may also file weekly claims on the telephone by calling 1-800-593-7660 and selecting the option to file weekly claims.

    Under certain conditions, you may be required to complete a weekly paper claim. Paper claims are required for Dislocated Worker Benefits, WorkShare, Trade, Extended Benefits and when your employer provides you with a partial unemployment form (B-9) to complete and submit to the claims center.

    Paper claims can be mailed or faxed to one of the claim centers posted on the back page of the blue booklet that is mailed to you when you establish a new claim for benefits.

    Filing a Claim

    Billions of dollars. That’s the amount of unclaimed money in the United States waiting to be given back to the rightful owners. The money includes forgotten apartment security deposits, uncashed overtime checks and lost insurance refunds, and it’s sitting there, waiting for you.

    Banks and other businesses are required to turn unclaimed money over to the states for safekeeping. The 50 states plus the District of Columbia have set up a free website you can use to see whether there are any forgotten funds in your name.

    At the crossroads of America, in New York City’s Times Square, we set up our “GMA” unclaimed money headquarters.

    Soon, we are in business and I am either a hero or a zero.

    At first, we are sweating — actually freezing — as we come up with nothing. Nada.

    But then, along comes a lucky lady.

    Motorola owes her money and the state of New York is sending a check.

    It’s important to check every state where you have lived. And every name you’ve ever lived under. Not all state websites tell you how much money you have coming.

    Thirty-two minutes into our experiment, we hit a streak.

    We found modest amounts of money for several people in a row.

    Then a woman approached us who suspected there was unclaimed money in her husband’s name. Sure enough, there were funds waiting for him from sort of unused cashier’s check.     Miscellaneous Money If you are searching for things such as forgotten apartment security deposits, uncashed overtime checks, lost insurance refunds or abandoned safe deposit boxes, your first stop is the states. The National Association of Unclaimed Property Administrators (NAUPA) has set up a free website at www.unclaimed.org that will link you to the appropriate department in your state that holds the funds.

    Unclaimed Savings Bonds It’s easy for savings bonds to go unclaimed because they take 30 to 40 years to mature. That’s why the Treasury Department has set up a simple search website, available HERE, where you can find forgotten bonds by typing in your social security number. Certain bonds are not listed online and require a hand search. You can read about them at the same Treasury link.

    Everybody looks forward to getting an income tax refund check, but if yours didn’t arrive, what do you do? The IRS now provides a “Where’s my Refund?” feature on its website. You can look up your missing check by entering the amount you are owed, plus your social security number. CLICK HERE.

    Lost Life Insurance Policies

    The proceeds of lost life insurance policies may turn up in your state search. If not, and you suspect you are the beneficiary of a loved one’s lost life insurance policy, you can hire a company called MIB Solutions to search for you. MIB is a private company that houses life insurance application information for much of the industry. It costs $75 to search. Go to www.policylocator.com for more information.

    If you didn’t collect your money when your bank went under, chances are your account was insured by the Federal Deposit Insurance Corporation (FDIC). The good news, in that case, is the FDIC is holding your money, and you can find out HERE.

    Failed Accounts In a Credit Union

    If your money was in a credit union as it failed, visit the National Credit Union Association (NCUA) HERE to track down your money.

    If your company still exists, or has been bought out, you need to approach the company directly. If you are owed a pension from a company that went under, there is a federal agency, the Pension Benefit Guaranty Corporation (PBGC), that safeguards private pensions. You can track down your pension HERE on the PBGC website.

    The Employee Benefits Security Administration (EBSA), is the Federal agency charged with making sure retirement money is reunited with its rightful owners. The EBSA sometimes even sues to seize retirement money. You can utilize the agency’s services by clicking HERE.

    Sometimes when people leave a job, they leave behind a 401(k) as well. If the company goes out of business, that only compounds the confusion. Fortunately, companies that administer 401(k) plans have teamed up to create a search engine you can use to track down your 401(k).

    Getting Authorization for inpatient hospital visit

    PRIOR AUTHORIZATION CERTIFICATION EVALUATION REVIEW (PACER)

    Reconsiderations The attending physician/dentist or the hospital may request reconsideration of the adverse determination of the ACRC regarding the need for admission, readmission, transfer, or continued stay. This reconsideration right applies regardless of the current hospitalization status of the beneficiary. Reconsiderations must be requested within three business days of the adverse determination. (Refer to the Directory Appendix for ACRC contact information.) If requested by the ACRC, the provider must provide written documentation. The provider is notified of the reconsideration decision within one business day of receipt of the request or the date of receipt of written documentation. If the initial adverse determination is overturned, the adverse determination is considered null and void. If the initial adverse determination is upheld or is modified in such a manner that some portion of the hospital care is not authorized, the hospital is liable for the cost of care provided from the date of the initial determination, unless this determination is overturned in the Medicaid appeals

    If the ACRC does not authorize the admission or the continued stay for an admission and the beneficiary remains in the hospital for one or more days after Medicaid payment is not authorized, the hospital is at risk of Medicaid nonpayment for those days.     Rival countries have wrangled over territory in the South China Sea for centuries, but tension has steadily increased in recent years.

    China, Vietnam, the Philippines, Taiwan, Malaysia and Brunei all have competing claims.

    China has backed its expansive claims with island-building and naval patrols. The US says it does not take sides in territorial disputes, but has sent military ships and planes near disputed islands, calling them “freedom of navigation” operations to ensure access to key shipping and air routes.

    Both sides have accused each other of “militarising” the South China Sea.

    There are fears that the area is becoming a flashpoint, with potentially serious global consequences.

    What is the argument about?

    It is a dispute over territory and sovereignty over ocean areas, and the Paracels and the Spratlys – two island chains claimed in whole or in part by a number of countries.

    Alongside the fully fledged islands, there are dozens of rocky outcrops, atolls, sandbanks and reefs, such as the Scarborough Shoal.

    Why are they worth arguing over?

    Although largely uninhabited, the Paracels and the Spratlys may have reserves of natural resources around them. There has been little detailed exploration of the area, so estimates are largely extrapolated from the mineral wealth of neighbouring areas.

    The sea is also a major shipping route and home to fishing grounds that supply the livelihoods of people across the region.

    Who claims what?

    China claims by far the largest portion of territory – an area defined by the “nine-dash line” which stretches hundreds of miles south and east from its most southerly province of Hainan.

    Beijing says its right to the area goes back centuries to when the Paracel and Spratly island chains were regarded as integral parts of the Chinese nation, and in 1947 it issued a map detailing its claims. It showed the two island groups falling entirely within its territory. Those claims are mirrored by Taiwan.

    However, critics say China has not clarified its claims sufficiently – and that the nine-dash line that appears on Chinese maps encompassing almost the entirety of the South China Sea includes no coordinates.

    It is also not clear whether China claims only land territory within the nine-dash line, or all the territorial waters within the line as well.

    Vietnam hotly disputes China’s historical account, saying China had never claimed sovereignty over the islands before the 1940s. Vietnam says it has actively ruled over both the Paracels and the Spratlys since the 17th Century – and has the documents to prove it.

    The other major claimant in the area is the Philippines, which invokes its geographical proximity to the Spratly Islands as the main basis of its claim for part of the grouping.

    Both the Philippines and China lay claim to the Scarborough Shoal (known as Huangyan Island in China) – a little more than 100 miles (160km) from the Philippines and 500 miles from China.

    Malaysia and Brunei also lay claim to territory in the South China Sea that they say falls within their economic exclusion zones, as defined by UNCLOS – the United Nations Convention on the Law of the Sea.

    Brunei does not claim any of the disputed islands, but Malaysia claims a small number of islands in the Spratlys.

    Recent flashpoints

    The most serious trouble in recent decades has flared between Vietnam and China, and there have also been stand-offs between the Philippines and China. Some of the incidents include:

    • In 1974 the Chinese seized the Paracels from Vietnam, killing more than 70 Vietnamese troops.
    • In 1988 the two sides clashed in the Spratlys, with Vietnam again coming off worse, losing about 60 sailors.
    • In early 2012, China and the Philippines engaged in a lengthy maritime stand-off, accusing each other of intrusions in the Scarborough Shoal.
    • Unverified claims that the Chinese navy sabotaged two Vietnamese exploration operations in late 2012 led to large anti-China protests on Vietnam’s streets.
    • In January 2013, Manila said it was taking China to a UN tribunal under the auspices of the UN Convention on the Laws of the Sea, to challenge its claims.
    • In May 2014, the introduction by China of a drilling rig into waters near the Paracel Islands led to multiple collisions between Vietnamese and Chinese ships.

    Have they tried to reach a resolution?

    China prefers bilateral negotiations with the other parties. But many of its neighbours argue that China’s relative size and clout give it an unfair advantage.

    Some countries have argued that China should negotiate with Asean (the Association of South East Asian Nations), a 10-member regional grouping that consists of Thailand, Indonesia, Malaysia, the Philippines, Singapore, Brunei, Laos, Vietnam, Myanmar and Cambodia.

    However, China is opposed to this, while Asean is also divided over how to resolve the dispute.

    The Philippines has sought international arbitration instead. In 2013, it announced it would take China to an arbitration tribunal under the auspices of the UN Convention on the Laws of the Sea, to challenge its claims.

    In July 2016, the tribunal backed the Philippines’ case, saying China had violated the Philippines’ sovereign rights.

    China had boycotted the proceedings, and called the ruling “ill-founded”. It says it will not be bound by it.

    Unemployment Benefits: Contesting an Employee s Claim

    Not everyone who’s out of work is entitled to unemployment benefits. There are a couple of factors that dictate whether a former employee will receive unemployment benefits: the circumstances of the employee’s departure and whether the employer contests the employee’s claim. This means your company has a lot of power over whether a worker will receive unemployment benefits. If a former employee files a claim, your company will need to decide whether or not to contest it.

    Employees are eligible for unemployment benefits only if they are out of work through no fault of their own. This rule works differently depending on whether the employee quit, was laid off, or was fired.

    An employee who loses a job through a layoff or reduction in workforce is always eligible for unemployment benefits.

    Fired employees can claim unemployment benefits if they were terminated because of financial cutbacks or because they were not a good fit for the job for which they were hired. They can also receive unemployment benefits if the employer had a good reason to fire the employee, such as being late for work several times, but the infractions were relatively minor, unintentional, or isolated.

    On the other hand, in most states an employee who is fired for misconduct will not receive unemployment benefits. Although you might think that any action that leads to termination should constitute misconduct, the unemployment laws don’t look at it that way. Not all actions that result in termination are serious enough to qualify as misconduct and justify denying benefits to a terminated worker.

    What qualifies as misconduct that will disqualify an employee from receiving unemployment benefits? Generally speaking, an employee engages in misconduct by willfully doing something that substantially injures the company’s interests. For example, revealing trade secrets or sexually harassing coworkers is typically the type of misconduct that renders the employee ineligible to collect unemployment benefits. Other common types of disqualifying misconduct include chronic tardiness, numerous unexcused absences, extreme insubordination, intoxication on the job, and dishonesty.

    Common actions that often result in firing — but do not constitute misconduct — include poor performance because of lack of skills, good faith errors in judgment, inefficient work habits, an unpleasant personality, poor relations with coworkers, or off-work conduct that does not have an impact on the employer’s interests. An employee fired for any of these reasons will usually be allowed to collect unemployment benefits.

    It is important to remember that what qualifies as misconduct is a matter of interpretation and degree. Annoying one coworker might not be considered misconduct that will disqualify an employee from receiving unemployment benefits, but intentionally engaging in actions that anger an entire department, even after repeated warnings, might be considered disqualifying misconduct.

    If an Employee Quits

    An employee who quits or resigns from a job will be eligible for benefits only if the employee resigned for “good cause.” A good reason for quitting a job, such as job dissatisfaction, is not necessarily good cause. The law requires the employee’s reason for leaving to be “compelling” — that is, the worker would have suffered some sort of harm or injury by staying. Put another way, the reason the employee left must be the sort that would have made any reasonable person leave.

    If an employee leaves a job because of intolerable working conditions (such as being sexually harassed) or because of being offered the opportunity to quit in lieu of being fired, most states would allow the worker to collect unemployment benefits. Similarly, leaving a job because it poses a serious threat to the worker’s health or safety is usually good cause. On the other hand, most states would not accept leaving a job because it doesn’t offer opportunities for career advancement as a good cause, and it won’t make a worker eligible for unemployment benefits.

    Should Your Company Contest the Claim?

    Your state’s unemployment office — not your company — will ultimately decide whether a former employee can receive unemployment benefits. You do, however, have the option of contesting an employee’s application for unemployment benefits, and that option gives your company a great deal of power. In California, for example, the unemployment board presumes that a terminated employee did not engage in misconduct that would disqualify the employee from getting unemployment benefits unless the employer contests the unemployment claim. Thus, in California, terminated employees who claim unemployment benefits receive them unless the former employer contests the claim.

    Remember, there is no reason — and there are no grounds — to contest an unemployment claim if the employee was laid off. There are also no grounds to contest the claim if the employee did not engage in misconduct but was fired for lesser reasons — for instance, for sloppy work, carelessness, poor judgment, or the inability to learn new skills.

    Even if an employee engages in misconduct, your company might want to give up its right to contest an unemployment insurance claim as part of a severance package, especially if the fired employee seems likely to sue. In other words, your company would agree not to contest unemployment benefits and the employee would agree not to sue your company.

    Your company should contest a claim only if it has grounds to do so — meaning that the employee engaged in serious misconduct or quit without a compelling reason. And even then, your company should also have a good, practical reason to contest. Employers typically fight unemployment claims for one of two reasons:

    • The employer is concerned that their unemployment insurance rates may increase. After all, the employer (not the employee) pays for unemployment insurance. The amount the employer pays toward unemployment insurance is based in part on the number of claims made against the employer by former employees.
    • The employer is concerned that the employee plans to file a wrongful termination action. The unemployment application process can be valuable in discovering the employee’s side of the story, and it can also provide an excellent opportunity for gathering evidence — both from the employee and from witnesses.

    If your company plans to contest an unemployment compensation claim, proceed with caution. These battles not only cost time and money, but they also ensure that the former employee will become an enemy. The employee might even file a wrongful termination lawsuit that otherwise could have been avoided. And if the fired worker has friends who remain on the job, they too may doubt and distrust your company’s tactics.

    Before making any decisions, you might want to do some research by contacting your state’s unemployment office for specific information about the law in your state. This office can tell you what effect a successful unemployment benefit claim will have on your company’s rates. If it’s relatively small, backing off might be a good idea.

    Tips For Filing A Car Insurance Claim

    If you drive, chances are that you will eventually be in an accident or have another incident that damages your car. You will need to follow certain steps to file a claim with your car insurance company to get reimbursement.

    Claim types include bodily injury claims for medical expenses and property damage claims to vehicle repairs and stolen property. Read on to find out how to file a car insurance claim.

    When to File a Claim

    Contact your car insurance company as soon as possible following a car collision. You may also need to file a claim if your vehicle is:

    • Vandalized.
    • Stolen.
    • Damaged from a non-accident-related event, such as fire or severe weather.

    Your auto insurance agent’s or insurance company’s telephone number is listed on the front of your insurance card. However, you may also be able to find it online and in your actual policy documents.

    Information You’ll Need

    When you contact your company to file a claim, make sure to provide the following details:

    • Your full name and policy number.
    • The start and end date of your policy.
    • Date and time of the incident.
    • Names, addresses, and phone numbers of all drivers, passengers, and witnesses.
    • Driver’s license and license plate numbers for all drivers.

    Your insurance company might also require you to complete a sworn statement that details the events of the auto accident (i.e. weather conditions, time of day or night that the incident occurred, etc.). Make sure also that you note any personal harm you notice and take photos of the damage.

    Five Basic Steps of Filing a Claim

    Once you contact the car insurance company, your case will go through the following steps.

    1. Case assignment to a professional at your car insurance company who will work on your claim.
    2. Meeting with your claims professional to discuss your coverages and the situation.
    3. Evaluation by your claims professional, which can include inspecting your car, examining evidence of any injury claims, and an initial payment.
    4. Resolution of the case, including payments as appropriate.
    5. Closing of the case.

    Since you are responsible for protecting your own property, your car insurance company may require you to make temporary fixes to your car while you are waiting for car repairs. These can help prevent further damage. However, be sure not to make permanent repairs before you are approved, because these may not be reimbursed.

    Make sure to keep receipts for expenses as you fix your car and seek medical treatment.

    • Take photos of the scene and your injuries.
    • File a police report and request a copy.
    • Visit a doctor immediately.
    • Write down everything related to your injury, including any work time or activities you may have missed due to the injury.

    Property Damage Claims

    Your ability to file a claim for vehicle repairs depends on the cause of the damage and the coverages that you have.

    Liability property damage coverage, which is required in many states, does not cover your own car. It will only pay for the damages you cause to another driver’s vehicle in an accident that you cause.

    The two major coverage types that pay for damage to your vehicle include:

    • Collision coverage, which pays for damages to your vehicle after a collision.
    • Comprehensive coverage, which will cover damages to your car occurring from events that aren’t related to accidents.
      • Examples include vandalism and severe weather.

    Other coverages that will cover property damage include:

    Handling a Property Damage Claim

    When you file a physical damage claim, an insurance adjuster will inspect your vehicle and estimate how much it will cost to repair the damage. You will then receive a check from the insurance company that takes into account the auto deductible you have chosen for your policy.

    If your vehicle has been damaged, follow these steps in order to have your car insurance claim processed effectively:

    • Report the damage to your vehicle to your insurance company immediately.
      • Depending upon your insurance company’s policies, you can do this online, by calling your insurance agent during regular business hours, or by calling the company’s claims division.
    • If another driver was involved, exchange insurance information.
    • Allow your insurance company to inspect the vehicle before repairs are made.
    • Protect the vehicle so no further damages are caused and losses are limited.
      • For example, if the accident causes the fender to rub against the vehicle tire, you should have this repair made immediately so the tire is not further damaged. If you don’t make the repair, your car insurance company can refuse to pay for any additional damages.
    • Save all receipts and any other documentation you receive to give to your insurance company as part of your claim.
    • Provide any requested information the insurance company needs as part of its investigation. If you refuse to cooperate, your claim could be denied.

    Why Waiting Is Torture

    SOME years ago, executives at a Houston airport faced a troubling customer-relations issue. Passengers were lodging an inordinate number of complaints about the long waits at baggage claim. In response, the executives increased the number of baggage handlers working that shift. The plan worked: the average wait fell to eight minutes, well within industry benchmarks. But the complaints persisted.

    Puzzled, the airport executives undertook a more careful, on-site analysis. They found that it took passengers a minute to walk from their arrival gates to baggage claim and seven more minutes to get their bags. Roughly 88 percent of their time, in other words, was spent standing around waiting for their bags.

    So the airport decided on a new approach: instead of reducing wait times, it moved the arrival gates away from the main terminal and routed bags to the outermost carousel. Passengers now had to walk six times longer to get their bags. Complaints dropped to near zero.

    This story hints at a general principle: the experience of waiting, whether for luggage or groceries, is defined only partly by the objective length of the wait. “Often the psychology of queuing is more important than the statistics of the wait itself,” notes the M.I.T. operations researcher Richard Larson, widely considered to be the world’s foremost expert on lines. Occupied time (walking to baggage claim) feels shorter than unoccupied time (standing at the carousel). Research on queuing has shown that, on average, people overestimate how long they’ve waited in a line by about 36 percent.

    This is also why one finds mirrors next to elevators. The idea was born during the post-World War II boom, when the spread of high-rises led to complaints about elevator delays. The rationale behind the mirrors was similar to the one used at the Houston airport: give people something to occupy their time, and the wait will feel shorter. With the mirrors, people could check their hair or slyly ogle other passengers. And it worked: almost overnight, the complaints ceased.

    The drudgery of unoccupied time also accounts in large measure for the popularity of impulse-buy items, which earn supermarkets about $5.5 billion annually. The tabloids and packs of gum offer relief from the agony of waiting.

    Our expectations further affect how we feel about lines. Uncertainty magnifies the stress of waiting, while feedback in the form of expected wait times and explanations for delays improves the tenor of the experience.

    And beating expectations buoys our mood. All else being equal, people who wait less than they anticipated leave happier than those who wait longer than expected. This is why Disney, the universally acknowledged master of applied queuing psychology, overestimates wait times for rides, so that its guests — never customers, always guests — are pleasantly surprised when they ascend Space Mountain ahead of schedule.

    This is a powerful ploy because our memories of a queuing experience, to use an industry term, are strongly influenced by the final moments, according to research conducted by Ziv Carmon, a professor of marketing at the business school Insead, and the behavioral economist Daniel Kahneman. When a long wait ends on a happy note — the line speeds up, say — we tend to look back on it positively, even if we were miserable much of the time. Conversely, if negative emotions dominate in the final minutes, our retrospective audit of the process will skew toward cynicism, even if the experience as a whole was relatively painless.

    Professors Carmon and Kahneman have also found that we are more concerned with how long a line is than how fast it’s moving. Given a choice between a slow-moving short line and a fast-moving long one, we will often opt for the former, even if the waits are identical. (This is why Disney hides the lengths of its lines by wrapping them around buildings and using serpentine queues.)

    Perhaps the biggest influence on our feelings about lines, though, has to do with our perception of fairness. When it comes to lines, the universally acknowledged standard is first come first served: any deviation is, to most, a mark of iniquity and can lead to violent queue rage. Last month a man was stabbed at a Maryland post office by a fellow customer who mistakenly thought he’d cut in line. Professor Larson calls these unwelcome intrusions “slips” and “skips.”

    The demand for fairness extends beyond mere self-interest. Like any social system, lines are governed by an implicit set of norms that transcend the individual. A study of fans in line for U2 tickets found that people are just as upset by slips and skips that occur behind them, and thus don’t lengthen their wait, as they are by those in front of them.

    Surveys show that many people will wait twice as long for fast food, provided the establishment uses a first-come-first-served, single-queue ordering system as opposed to a multi-queue setup. Anyone who’s ever had to choose a line at a grocery store knows how unfair multiple queues can seem; invariably, you wind up kicking yourself for not choosing the line next to you moving twice as fast.

    But there’s a curious cognitive asymmetry at work here. While losing to the line at our left drives us to despair, winning the race against the one to our right does little to lift our spirits. Indeed, in a system of multiple queues, customers almost always fixate on the line they’re losing to and rarely the one they’re beating.

    Fairness also dictates that the length of a line should be commensurate with the value of the product or service for which we’re waiting. The more valuable it is, the longer one is willing to wait for it. Hence the supermarket express line, a rare, socially sanctioned violation of first come first served, based on the assumption that no reasonable person thinks a child buying a candy bar should wait behind an old man stocking up on provisions for the Mayan apocalypse.

    Americans spend roughly 37 billion hours each year waiting in line. The dominant cost of waiting is an emotional one: stress, boredom, that nagging sensation that one’s life is slipping away. The last thing we want to do with our dwindling leisure time is squander it in stasis. We’ll never eliminate lines altogether, but a better understanding of the psychology of waiting can help make those inevitable delays that inject themselves into our daily lives a touch more bearable. And when all else fails, bring a book.

    Rival countries have wrangled over territory in the South China Sea for centuries, but tension has steadily increased in recent years.

    China, Vietnam, the Philippines, Taiwan, Malaysia and Brunei all have competing claims.

    China has backed its expansive claims with island-building and naval patrols. The US says it does not take sides in territorial disputes, but has sent military ships and planes near disputed islands, calling them “freedom of navigation” operations to ensure access to key shipping and air routes.

    Both sides have accused each other of “militarising” the South China Sea.

    There are fears that the area is becoming a flashpoint, with potentially serious global consequences.

    What is the argument about?

    It is a dispute over territory and sovereignty over ocean areas, and the Paracels and the Spratlys – two island chains claimed in whole or in part by a number of countries.

    Alongside the fully fledged islands, there are dozens of rocky outcrops, atolls, sandbanks and reefs, such as the Scarborough Shoal.

    Why are they worth arguing over?

    Although largely uninhabited, the Paracels and the Spratlys may have reserves of natural resources around them. There has been little detailed exploration of the area, so estimates are largely extrapolated from the mineral wealth of neighbouring areas.

    The sea is also a major shipping route and home to fishing grounds that supply the livelihoods of people across the region.

    Who claims what?

    China claims by far the largest portion of territory – an area defined by the “nine-dash line” which stretches hundreds of miles south and east from its most southerly province of Hainan.

    Beijing says its right to the area goes back centuries to when the Paracel and Spratly island chains were regarded as integral parts of the Chinese nation, and in 1947 it issued a map detailing its claims. It showed the two island groups falling entirely within its territory. Those claims are mirrored by Taiwan.

    However, critics say China has not clarified its claims sufficiently – and that the nine-dash line that appears on Chinese maps encompassing almost the entirety of the South China Sea includes no coordinates.

    It is also not clear whether China claims only land territory within the nine-dash line, or all the territorial waters within the line as well.

    Vietnam hotly disputes China’s historical account, saying China had never claimed sovereignty over the islands before the 1940s. Vietnam says it has actively ruled over both the Paracels and the Spratlys since the 17th Century – and has the documents to prove it.

    The other major claimant in the area is the Philippines, which invokes its geographical proximity to the Spratly Islands as the main basis of its claim for part of the grouping.

    Both the Philippines and China lay claim to the Scarborough Shoal (known as Huangyan Island in China) – a little more than 100 miles (160km) from the Philippines and 500 miles from China.

    Malaysia and Brunei also lay claim to territory in the South China Sea that they say falls within their economic exclusion zones, as defined by UNCLOS – the United Nations Convention on the Law of the Sea.

    Brunei does not claim any of the disputed islands, but Malaysia claims a small number of islands in the Spratlys.

    Recent flashpoints

    The most serious trouble in recent decades has flared between Vietnam and China, and there have also been stand-offs between the Philippines and China. Some of the incidents include:

    • In 1974 the Chinese seized the Paracels from Vietnam, killing more than 70 Vietnamese troops.
    • In 1988 the two sides clashed in the Spratlys, with Vietnam again coming off worse, losing about 60 sailors.
    • In early 2012, China and the Philippines engaged in a lengthy maritime stand-off, accusing each other of intrusions in the Scarborough Shoal.
    • Unverified claims that the Chinese navy sabotaged two Vietnamese exploration operations in late 2012 led to large anti-China protests on Vietnam’s streets.
    • In January 2013, Manila said it was taking China to a UN tribunal under the auspices of the UN Convention on the Laws of the Sea, to challenge its claims.
    • In May 2014, the introduction by China of a drilling rig into waters near the Paracel Islands led to multiple collisions between Vietnamese and Chinese ships.

    Have they tried to reach a resolution?

    China prefers bilateral negotiations with the other parties. But many of its neighbours argue that China’s relative size and clout give it an unfair advantage.

    Some countries have argued that China should negotiate with Asean (the Association of South East Asian Nations), a 10-member regional grouping that consists of Thailand, Indonesia, Malaysia, the Philippines, Singapore, Brunei, Laos, Vietnam, Myanmar and Cambodia.

    However, China is opposed to this, while Asean is also divided over how to resolve the dispute.

    The Philippines has sought international arbitration instead. In 2013, it announced it would take China to an arbitration tribunal under the auspices of the UN Convention on the Laws of the Sea, to challenge its claims.

    In July 2016, the tribunal backed the Philippines’ case, saying China had violated the Philippines’ sovereign rights.

    China had boycotted the proceedings, and called the ruling “ill-founded”. It says it will not be bound by it.

    How to Apply for Florida Unemployment Benefits

    In these doubtful times, being jobless is a fact of life that many people have to bear. In Florida, like the rest of the nation, many people have to file for jobless benefits for the first time in their lives. Fortunately for the recently unemployed of Florida, the process of filing for unemployment benefits is quite easy. However, you can look ahead to wait about 4 weeks until you collect your first check, so you will want to begin the process on your first day of unemployment.

    This page has been created as a resource to help you in your filing for Florida unemployment compensation benefits. After reading the following, you will know if you meet the criteria for unemployment benefits, how to apply for benefits, as well as when to expect getting unemployment benefits.

    Determine if You are Eligible

    In order to open an unemployment claim to obtain benefits, you must first decide if you are entitled. To collect benefits in Florida, you must have worked lately.

    You must have been worked all through 4 of the 5 immediate preceding quarters to your claim. Quarters begins in the following months: January, April, July and October. You must have been compensated during 2 of those 5 quarters.

    You need to have earned $3400 during those 4 quarters.

    Apart from being entitled from a monetary perspective, you also have to be eligible based on disconnection from your last job. You just have:

    • Been separated at no fault of your own (you were not let go for performance issues, for example)
    • You must not have give up on your own
    • You must be obtainable and willing to work. You must be keenly searching for new employment opportunities

    Before filing your claim on-line, collect the following information to help you in completing the application:

    1. Your Social Security number;
    2. The names, addresses, and phone numbers of all your employers during the last 18 months;
    3. The dates you worked and total earnings from each employer;
    4. Gross earnings for this week since 12:01AM Sunday;
    5. Driver’s License, State Identification, voter registration number or other type of ID that could verify your identity;
    6. The name and local number of your labor union hall,
    7. if applicable;
    8. If you are not a U.S. citizen, your Alien Registration Number and work permit expiration date;
    9. Your DD-214 form (if you were in the military within the last 2 years);
    10. Form SF-50 or Form SF-8 and check stubs or W-2 proof of earnings (if you were a federal employee)

    Filing a Claim

    Make a decision how you want to file your claim. Individuals may use one of several formats to apply for unemployment compensation in Florida.

    • File a claim online by visiting the agency s website at FloridaJobs.org
    • Call the agency 1-800-204-2418 during normal business hours, Monday through Friday. (The number to the unemployment office is often busy or unmanned, so filing online may be the best course of action).

    Filing On-Line

    The Internet application is available 24 hours a day, seven days a week. If you do not have access to the Internet at your residence, you may need to use a public computer to file your reemployment claim. You may want to try a public library, college or university library or use a computer at your local One-Stop Career Center.

    The One-Stop Career Center nearest you may have computer facilities that you can use to file your claim on the Internet. The One-Stop Career Center is also your best source for job placement services in your area.

    To file your claim on-line

    Go to the website and locate the Job Seekers box on the left side of the page and select File an Reemployment Assistance Claim.

    This site permits Floridians and former Florida workers to file claims for reemployment assistance using Internet. The process takes 30 minutes to one hour from start to finish.

    The date your application is finished will decide the date your benefits will commence.

    Filing on Telephone

    Claiming Unemployment in Florida can also be done by phone at 1-800-204-2418 (Monday Friday 7:30am to 7PM and Saturdays 8am 4pm). Claiming online is recommended as the wait times to speak to a call center agent can reach an hour because of increased volume of calls Florida State is handling.

    Skills Review

    Complete an Initial Skills Review. In order to receive benefits, you must complete a skills assessment review comprised of 3 modules designed to determine your job skill levels. This information will be used by Regional Workforce Board to develop a plan for workforce development in your region of the state.

    You must complete all 3 parts within 17 days of your claim’s effective date or else payments will be delayed or denied.

    What to Wait for after you File Your Unemployment Claim?

    About a week after applying, you will obtain a letter stating if you are eligible and, if so, how much you will collect weekly as your benefit. In case you do not qualify for benefits, the letter will clarify why and tell you what you must do if you wish to appeal the verdict.

    Once you have qualified to collect benefits, you must claim your weekly benefits every 2 weeks. The initial letter you obtain will give you a date on which you must call or go online and answer a few questions about your continued eligibility. Failure to reclaim your benefits every two weeks will hold up the payment of benefits to you.

    Moreover, in the state of Florida, the first week of unemployment is not considered eligible for unemployment benefits. As such, your first date to claim your weekly benefits will be 3weeks after you initially claimed unemployment benefits, and then the benefits paid out will only be for 2 weeks.

    How one man turns annoying cold calls into cash

    A man targeted by marketing companies is making money from cold calls with his own higher-rate phone number.

    In November 2011 Lee Beaumont paid £10 plus VAT to set up his personal 0871 line – so to call him now costs 10p, from which he receives 7p.

    The Leeds businessman told BBC Radio 4’s You and Yours programme that the line had so far made £300.

    Phone Pay Plus, which regulates premium numbers, said it strongly discouraged people from adopting the idea.

    Mr Beaumont came up with the plan when he grew sick of calls offering to help him reclaim payment protection insurance (PPI), or install solar panels.

    He said: “I don’t use my normal Leeds number for anyone but my friends and family.”

    Once he had set up the 0871 line, every time a bank, gas or electricity supplier asked him for his details online, he submitted it as his contact number.

    He added he was “very honest” and the companies did ask why he had a such a number.

    He told the programme he replied: “Because I’m getting annoyed with PPI phone calls when I’m trying to watch Coronation Street so I’d rather make 10p a minute.”

    He said almost all of the companies he dealt with were happy to use it and if they refused he asked them to email.

    The number of calls received by Mr Beaumont has fallen from between 20 and 30 a month to just 13 last month.

    Because he works from home, Mr Beaumont has been able to increase his revenue by keeping cold callers talking – asking for more details about their services.

    Annoying calls

    Citizens Advice commissioned a survey about unwanted PPI calls.

    One in four of those who had received unwanted calls said this had interrupted family meals.

    Calls remain the most common form of contact about PPI claims, followed by automated landline messages and text messages to mobiles.

    Anyone who believes that they have been mis-sold PPI can make a claim directly to their provider.

    He admitted the scheme had changed his attitude, saying: “I want cold calls”, and that he had moved on to encouraging companies to make contact.

    After a recent problem with his online shopping, he declined to call an 0845 number but posted his number on Twitter in the knowledge that the number could be picked up by marketing companies.

    But the premium number regulator Phone Pay Plus says the public should think twice before setting up their own lines.

    They say phone line providers must meet consumer protection standards, which include transparency, fairness and complaint handling, which would mean clearly setting out the cost of each call to any organisation that rang.

    They told You and Yours: “Premium rate numbers are not designed to be used in this way and we would strongly discourage any listeners from adopting this idea, as they will be liable under our code for any breaches and subsequent fines that result.”

    A survey for charity Citizens Advice found that two-thirds of those asked had received unwanted calls, texts, emails or letters about PPI mis-selling.

    More than half said that they had been contacted more than 10 times in the past year.

    Make sure that you’re healthy! ‘Control the controllable’ as they say: Eat a great diet, exercise, stop smoking, stop drinking, stop smoking the funny stuff! get more sleep. It may sound a little boring, but if you are over-indulging in any of these areas, it could stop IVF from working

    Cut down on the coffee. For women, NEW research shows that drinking more than 5 cups of coffee a day has a negative effective on you’re chances of getting pregnant with IVF. So hold back on those free refills!

    During IVF, if you produce more better size follicles you’ll have a higher chance of getting pregnant. The ideal situation is around 8-15 eggs at 16mm-22m in size. If you only have 3 then you need to consider this – should you freeze them go for another 2-3 stimulation cycles before fertilizing a good number of eggs in one go? This should increase you chances of success.

    Use a clinic that offer blastocyst transfers, but remember they aren’t always better that a good 3 day embryo! Also if you’re going to freeze some eggs for a later cycle – always use the vitrification freezing technique – it increases your chances of success.

    Be realistic, especially with age. Unexplained infertility affects thousands of couples every year. Sometimes it’s just nature’s way of saying – you’re getting a bit old for this! Between the ages of 43-45 it’s probably time to at least consider using donor eggs. Donors will be matched to you major physical characteristics, which helps bonding with your new child.

    Finally choose a fertility clinics that offer packages or discounts for multiple treatments. Why? Because you’ll probably need them! Some even offer refund guarantees. These allow women to try IVF three times. So even if it’s not successful, you still have enough money to try IVF with donor eggs. As we said at the beginning – it’s a numbers game!       This would be a very short article and specific to those who are interested in PMP certifications.

      For those not familiar with PMP this refers to the Project Management Professional certification, one of the Project Management Institute (PMI) certifications.

      Facts

      The following data is PMI officially published data (all data is up to the end of August 2011)

      1. The number of those holding the PMP Certification in the world is: 466,163
      2. The number of PMP earning their certification between 1 July 2010 and 30 June 2011 (12 months period) is: 48,273
      3. The monthly average number of new PMP in the 12 months period in previous note is: 4,023
      4. The number of PMP in July 2011 jumped from the average of 4,023 to 13,869
      5. The number of PMP in August 2011 (last month) also jumped from the average of 4,023 to 14,295
      6. In summary, in two months we have had close to 30,000 PMP about 60% of the full year before!

      Why the Change

      Obviously no one knows the exact answer. However, what we know is:

      1. The PMP exam changed at the end of August 2011
      2. Historically we know where there is an exam change the number of PMP candidates jump
      3. Usually the reason for the jump is that people and professionals who has been considering or studying for the PMP and delaying the exam, they make the push before the change since a change brings uncertainty.
